Click on the link for your degree program below. All instructions for self applying for graduation through SIS, checking your diploma name in SIS, and completing the graduation form (which must be signed by your advisor) are included in the graduation form.
Students enrolled in Ed.S, and doctoral programs must maintain a current Record of Progress Form, which should be started upon initial registration. Review the form each semester with your advisor to ensure you are meeting all program requirements.
Doctoral (Ed.D. & Ph.D.) students must submit their completed Record of Progress Forms to the Office of Admissions and Student Affairs at the same time they are turning in their dissertation materials for graduation.
Education Specialist (Ed.S.) Records of Progress forms are not submitted to the Office of Admissions, as they are used only for advising purposes.
